To sign up for services we need a Client Intake Form on file. You can download and print out an intake form below, pick one up at our office, or have us mail or email one to you.

- Mobile Meals Intake Form (for home-delivered meals in Victoria and DeWitt Counties)
- Congregate Meals Intake Form (for Connection Cafe Services)
2. Email your completed intake form to intake@mowstx.org or fax to 361.578.8111
Once received, we can assess your eligibility. For assistance call the Intake Coordinator at 361-576-2189 Option 2
